I'd rather finish the conversation I started with someone and then return a call if need be. I never ask someone to hold, I let the other person go through to voice mail, if it's important, they'll leave a message or they'll call back, simple. If it's someone you know, pop then a call afterwards, explain that you were in a call, they'll understand, if they don't, it's not someone you want to work with anyway ;) As far as meetings go, I dare someone to find me in a meeting with my mobile on, I wouldn't even go as far as setting it to silent, it goes off and stays off until I shake the final personsn hand good bye.
